What to bring to your first appointment:
We want to ensure your journey is as smooth as possible, so it’s important that you are prepared for your first consultation with us. Please review our appointment document checklist below.
- Referral letter from your GP or other specialist/ doctor
- Medicare card, DVA card, Pension Card
- Private Health Insurance Information
- A list of your medications and allergies
- Copies of all relevant test results (Ultrasounds, CT Scans, MRIs, Blood or urine tests)

For regional NSW patients:
We understand that specialist urogynaecology services may not be available near where you live. For this reason, Dr Chen provides Telehealth consultants for initial visits for patients who live >3hours away from Sydney. For those who will need to travel to Sydney to see Dr Chen, special government access scheme is able to provide you some financial assistance. Cancellation Policy:
You will not be charged for cancellations. But in the interest of allowing other women on our waiting list to have appointment times, we please ring the office in business hours at least 24hours prior to your appointment.
